PLEASE NOTE:

• Request minimum is $500,000. For requests under $500,000, apply to the CPL Grant Program.
• BEFORE submission, please print and review your proposal. You will receive confirmation of your submission via system generated email. If you do not receive a confirmation email, contact LSOHC staff at 651-284-6430.
